In vitro fertilization with TESE
In vitro fertilization is a highly complex assisted reproduction treatment.
It consists of the union of the ovum with a sperm in the laboratory –in vitro–, in order to obtain good quality embryos that can, after transfer to the mother's uterus, give rise to a pregnancy.

To carry out this procedure it is necessary to hormonally stimulate the patient in a controlled way and obtain her eggs, which are then fertilized in vitro in the laboratory, where they are cultured for a few days. One of these embryos is transferred back to the uterine cavity and the rest are cryopreserved for future use.

For whom is In Vitro Fertilization indicated?
In vitro fertilization is an alternative treatment when other fertility treatments have previously failed and in patients with disorders such as:

Women after several unsuccessful artificial inseminations.

Women with advanced endometriosis, with probable affection in the tubes and oocyte quality.

Older women with poor quality eggs.

Women with tubal injury or absence thereof.

Women with hydrosalpinx.

Couples in which a preimplantation genetic study is necessary.

Couples in which the man has poor sperm quality (moderate or severe male factor).
Testicular sperm extraction (TESE)
When the fertility problem comes after a vasectomy, the best option in order to obtain more chances of pregnancy is In Vitro Fertilization (IVF-ICSI). Trying to solve the obstruction through surgery is an alternative that is being abandoned given the poor results obtained, the aggressiveness of the technique (it is still a complex intervention) and the wait of months to check if the seminal quality obtained after the intervention is enough to get pregnant. In these cases, we opted for IVF-ICSI with extraction of sperm from the testis (IVF-ICSI with TESE) or from the epididymis. It is not possible with sperm extracted from testicles to carry out Artificial Insemination (AI) due to their low mobility.
Once the sperm are obtained, they are frozen for later use on the day of the follicular puncture.
